The Supported Living Group (SLG) provides Connecticut Home Care Program for Elders (CHCPE) and Personal Care Assistance (PCA) services designed to help eligible individuals remain safely and independently in their homes and communities.
Connecticut's CHCPE program provides home and community-based services to eligible adults age 65 and older who are at risk of nursing home placement and require assistance with critical daily activities. Available services may include personal care, companion and homemaker supports, care management, transportation, home-delivered meals, assistive technology, environmental accessibility adaptations, and other supports designed to promote independence at home.
