Program Manager

With a strong background in both healthcare and advocacy, Sarah brings a wealth of experience to her role as Program Manager with The Supported Living Group’s Avon region. Her professional journey has centered on supporting individuals with complex medical, behavioral, and neurological needs, both in healthcare settings and in educational systems.

Since joining SLG in 2024, Sarah has delivered high-quality, individualized services to adults navigating brain injury, physical disability, and co-occurring behavioral health challenges. She manages care for a diverse caseload, ensures 24/7 staff scheduling, and serves as a critical liaison between clinical teams, family members, and state funding partners. Sarah is deeply committed to maintaining the highest standards of participant safety, dignity, and voice, whether through care plan implementation, advocacy, or direct support.

Prior to her work with SLG, Sarah held leadership and strategic roles in the nonprofit and for-profit sectors. She served as Strategic Alliances Director with BuyerForesight, where she helped scale a financial services client to acquisition through targeted lead generation and insight-driven growth strategies.

Sarah is also a long-standing Special Education Lay Advocate, guiding families across multiple states through the complex world of IEPs, 504 Plans, and school transition planning. This background underscores her passion for person-centered service and systemic equity—values that drive her work every day at SLG.
